Registration fees include continental breakfasts, lunches, refreshment breaks, and syllabus materials.**
A cancellation fee of 25% will be assessed on refunds requested prior to July 21, 2009, and a 50% fee on refunds requested on July 21, 2009, through August 7, 2009. No refunds will be made after August 7, 2009. There is no charge for substitution.

** Please note an important change in how syllabus materials will be made available. To provide more current slides in the syllabus and as part of the growing “green initiative,” no printed syllabus containing slide sets will be available at this event. All slides will be available electronically prior to the meeting, and registrants will receive an active link with instructions on how to access the syllabus from our Web site for downloading and/or printing. It is suggested that you attend this event with your laptop, if possible. Power will be available on-site for your laptop, although Internet access will not be provided.
