A cancellation fee of 25% will be assessed on refunds requested prior to August 22, 2008, and a 50% fee on refunds requested on August 22, 2008, through September 5, 2008. No refunds will be made after September 5, 2008. There is no charge for substitution.

Eighth Annual Medical Oncology Fellows Forum:
Sunday, September 21, 2008 In addition to the Fall Oncology agenda, a special Medical Oncology Fellows Forum will be held; the focus of the forum will be the business aspects of oncology.
Scholarships are available to fellows; please complete a Fellows Forum Application from PER, and forward it with a letter of good standing from your Fellowship Director. Scholarships include meeting registration, coach airfare, and hotel accommodations (based on shared accommodations). Private accommodations are available for a fee of $105.50 plus tax per night payable to the hotel.
