In the PETACC-3 randomized phase III trial, the addition of irinotecan to adjuvant infusional 5-fluorouracil/leucovorin (irinotecan/LV5FU2) therapy failed to significantly improve disease-free survival, relapse-free survival, or overall survival compared to LV5FU2 alone in patients with stage III colon cancer.

Based on the final results of the MOSAIC phase III trial, adjuvant therapy with infusional 5-fluorouracil/leucovorin plus oxaliplatin (FOLFOX4) significantly improved both disease-free survival at 6 years and overall survival at 5 years compared to infusional 5-fluorouracil/leucovorin (LV5FU2) alone in patients with stage II or III colon cancer.

In a retrospective analysis, the use of isolated hepatic perfusion with a melphalan–containing regimen in patients with unresectable liver metastases from colorectal cancer that had or had not received previous systemic therapy was associated with a 2-year survival rate of 34%; preoperative CEA (< 30 ng/mL) and post – perfusion hepatic arterial infusion with floxuridine were significantly associated with improved patient survival.

A small, multicenter single arm phase II trial evaluating the combination of capecitabine, oxaliplatin, and irinotecan (XELOXIRI) as first-line therapy in patients with metastatic colorectal cancer reported an overall response rate of 67% and a median overall survival of 17.9 months, but was associated with a high incidence of diarrhea and neutropenia.
