Learning Objectives
Upon completion of this educational activity, you should be able to:
- Evaluate the efficacy of first-line chemotherapy options for patients with metastatic CRC
- Discuss the safety and efficacy considerations for the incorporation of targeted agents in first-line regimens for metastatic CRC
- Assess the advantages and disadvantages of chemotherapy-free intervals with regard to disease outcome in patients with CRC
Overview and Purpose
Overview and Purpose
First-line treatment of patients with metastatic colorectal cancer (CRC) requires careful consideration of prior therapy, comorbidities, potential for surgical resection, and tumor characteristics. Several combination chemotherapy regimens have demonstrated significant efficacy in the first-line setting, but the optimal choice is dependent on patient characteristics, preferences, and goals of therapy. The use of a temporary chemotherapy holiday versus continuation until disease progression is currently under debate and has the potential to affect long-term outcome and quality of life. The addition of agents targeting the vascular endothelial growth factor and epidermal growth factor receptor pathways has significantly improved progression-free survival in the first-line setting, although careful patient selection and consideration of toxicities is required to maximize the antitumor potential of these agents. Ongoing trials are exploring the appropriate therapeutic strategies for first-line metastatic CRC and will seek to improve individualization of therapy.
The purpose of this activity is to update physicians on the latest clinical data and treatment recommendations concerning the first-line treatment of a patient with metastatic CRC.
