Lecture Overview

Summary and Purpose

Much of the current era of targeted therapies has resulted from the lessons learned from our understanding of the molecular basis of chronic myeloid leukemia (CML) and the introduction of the first-generation Abl tyrosine kinase inhibitor (TKI), imatinib mesylate, into the clinic almost a decade ago. The lessons learned from Bcr-Abl tyrosine kinase inhibition in patients with CML have continued with an understanding of how CML cells have de novo or secondary resistance to TKIs via alterations such as the T315I mutation in the TK domain. Recently, the identification of an acquired point mutation in the JAK2 gene in patients with polycythemia vera, idiopathic myelofibrosis, and essential thrombocythemia has brought about discussion of the similarities of the role of JAK2 in these myeloproliferative disorders and the initiating events in CML via Bcr-Abl.

The purpose of this activity is to update physicians and others interested in hematologic malignancies on strategies for circumventing resistance to CML treatment with TKIs and on some of the molecular mechanisms involved in related myeloproliferative disorders.

Target Audience

This activity is intended for medical oncologists and hematologists involved in the care of patients with chronic myeloid leukemia and related myeloproliferative disorders. No specific skills or knowledge other than a basic training in oncology are required for successful participation in this activity

Learning Objectives

Upon completion of this educational activity, you should be able to:

  • Discuss the molecular mechanisms of how CML cells develop resistance to Bcr-Abl TKIs
  • Identify the significance of the T315I mutant and strategies to target that clone
  • Describe the characteristics associated with hypereosinophilic leukemia
